Abstract

بطريقة التثبيت الخارجي باستخدام نوع حديث من نظام التثبيت الخارجي متعدد مستويات التثبيت الفراغية   (R. A. L. C. A. M) في مشفى الأسد الجامعي وفي الوحدة المهنية للجراحة العظمية والرضية  وكانت هذه الآفات العظمية مؤلفة من كسور متنوعة. عشرة مرضى خزع عظم  تصحيحي ، ستة مرضى مفصل موهم ، خمسة مرضى, إيثاق مفصلي أربعة مرضى .

وكانت النتائج جيدة عند ثمانية عشر مريضاً وبنسبة مئوية قدرها (72%) موزعة على الشكل التالي : ست حالات من الكسور (60%) وجميع حالات خزع العظم التصحيحي (100%) وعند ثلاث حالات مفصل موهم (60/100) وثلاث حالات إيثاق مفصلي (75/100) .

ونتائج مقبولة عند سبعة مرضى وبنسبة قدرها (28%) موزعة على النحو التالي:

أربع حالات عند مرضى الكسور (40% ). وحالتان من بين حالات المفصل الموهم (40%). وحالة واحدة من بين حالات الإيثاق (25%) .

ووفقاً لهذه النتائج الجيدة المتعددة التي تثبت جدوى (م.خ.م.س)   كطريقة علاجية مثالية في معالجة الكسور المفتوحة عالية الطاقة, (درجة 3 حسب كامبل)  خاصة المتفتتة والمفصلية منها ، وكطريقة فعالة في معالجة بعض المشاكل التي تحدث أثناء المعالجة مثل عدم الاندمال وتشكل المفصل  الموهم, وسوء الاندمال (تزوي محور العظم) ومعالجة آفات القدم ,

نؤكد على أهمية  تأمين هذه الأجهزة وتعميمها على مستشفيات القطر .

 

A case study has been conducted on twenty five patients suffering from various orthopedic dysfunction. They have been treated at Al-Assad University Hospital, Department of and Orthopedics, using the R.A.L.C.A. M. These dysfunctions consisted of multiple fractures (10 Patients), osteotomies (6 patients), pseudoartrosis (5 patients) and artodeses (4 patients) Good results have been observed with 18 patients and with aratio of 72 %, distributed as follows : Six fractures 60 %, all Osteotomy cases 100 %, three pseudoartrosis 60 %, and three artrodeses 75 % .

Satisfactory results have been observed with seven patients and with a ratio of 28 %, distributed as Follow :

Four cases of multiple fractures, two cases of pseudoartrosis, and one case of artodesis .

These good results prove the validity of treatment of high-energy multiple fractures (level 3 as Campells, classification), especially fragmented bones. They also prove its efficiency in treating some surgical problems, like the unhealing of wounds, the formation of pseuoartosis, bone axiomatic displacement, foot deformity, etc .

We, therefore, emphasize the need for these equipments to be available at Syrian hospitals.
