دراسة سريرية وتشريحية مرضية لأربع عشرة حالة ورم لحمة في الجهاز المعدي المعوي (Gastrointestinal stromal tumor)

Abstract

تنشأ أورام اللحمة في الجهاز المعدي المعوي من الخلايا البينية لكاجال التي تلعب دورا في حركية الأمعاء وتشكل الأورام الناشئة منها 1% من الأورام الخبيثة لهذا الجهاز. يهدف البحث

إلى تسليط الضوء على هذه الأورام وتحديد عناصر التشخيص وميزات السلامة والخبث فيها .

تم في هذا البحث دراسة 14 حالة ورم لحمة في الجهاز المعدي المعوي – 8 من المعدة, 4 من الأمعاء الدقيقة, واحدة في المستقيم وواحدة في البطن من البريتوان. أجريت الدراسة على العينات المستأصلة جراحيا والمشخصة سريريا تحت عناوين مختلفة. اختلفت المظاهر العيانية من حيث التوضع في الجدار, بعضها أخذ شكل كتلة صلبة مع كيسات موضعة, والبعض الآخر كان كيسيا. تعددت المظاهر النسيجية وشملت: خلايا مغزلية, خلايا بشرانية, خلايا فاتحة وغيرها . شملت الطريقة النسيجية إجراء الملونات المناعية المناسبة وخاصة
(CD34, CD117  ) لتأكيد التشخيص.تمت الدراسة في مستشفى الملك فهد المركزي بجازان – السعودية.

Fourteen cases of gastrointestinal stromal tumors: 8 in the stomach, 4 in the small intestine, 1 in the rectum,  and 1 in the peritoneum have been studied macroscopically and microscopically (including immunohistochemical stains like CD117, CD34, Actin, Vimentin and S100). Macroscopically, the tumors were solid and cystic with focal necrosis and hemorrhage. Histologically, they revealed spindle, epithelial, clear and polymorphic cells. This  study, which was carried out in KFCH- Saudi Arabia,  would help offer criteria for diagnosing gastrointestinal stromal tumor and determining benignity and malignancy.
