نتائج تقنية Lich-Gregior كعلاج مضاد للجذر المثاني الحالبي عند الأطفال بين 3 و 6 سنوات " خبرة مشفى الأسد الجامعي باللاذقية"
Abstract
أجريت الدراسة في مستشفى الأسد الجامعي باللاذقية بين عامي 2000 و 2005, وشملت 26 مريضاً لديهم جذر مثاني حالبي عمرهم الوسطي 4.69 سنة ) تراوحت الأعمار بين 3 و6 سنة) . كان كل مرضانا من البنات. شخص الجذر لديهن بتصوير المثانة و الإحليل بالطريق الراجع أثناء الامتلاء و أثناء التبول . و عولجوا جراحياً بطريقة ليش غريغوار. في 58% منهم كان الجذر في الناحية اليمنى و في 31% كان الجذر في الناحية اليسرى وفي11% كان الجذر ثنائي الجانب.27% من المرضى كان لديهم جذر مثاني حالبي درجة II و 58% درجة III و19% درجة IV. وبعد 3 أشهرمن المعالجة أجري تصوير مثانة و إحليل بالطريق الراجع لكل المرضى فغاب الجذر عند 92% منهم و استمر الجذر عند الباقين ونسبتهم 8% و كانت درجته )Iولكن بمراقبة المرضى الناكسين لـ 3 أشهر أخرى زال الجذر بالعلاج المحافظ لوحده(., كما غاب الجذر في الجهة المقابلة عند مرضى الاصابة المزدوجة على الرغم من عدم إصلاحه أثناء العمل الجراحي مع العلم أننا قمنا في هذه الحالات بإصلاح الجذر الأشد درجة. و بمقارنة نتائجنا مع الدراسات العالمية و جدنا تقارباً كبيراً في نسب الشفاء.
This study was performed at Al assad University Hospital between 2000 and 2005 and included 26 patients with vesicoureteral Reflux. The mean age was 4.69 years (with age ranging between 3 and 6 years). All patients were females. They were diagnosed by voiding cystourethrography (VCUG) and surgically treated by Lich-Gregoir technique. The reflux was on the right side in 58% of cases, on the left side in 31% of the cases, and bilateral in 11%. 27% of patients had degree II vesicoureteral Reflux 58% had degree III, and 19% had degree IV. Following 3 months of treatment for all patients, V.C.U.G was carried out, and the results were as follows: absence of reflux in 92% of cases, persistence of reflux (degree I) in 8% of cases, absence of contra-lateral reflux, though we didn't do anything for it. The persistent reflux was treated through another 3 months only by conservative therapy. There is a similar percentage of reflux recovery in comparison with international studies.
