تأثير العمر والبدانة على العلاقة بين الداء السكري وضخامة البطين الأيسر
Abstract
هدف البحث دراسة ضخامة البطين الأيسر وتحديد نماذج هذه الضخامة عند مرضى الداء السكري النمط II وتحديد تأثير كل من العمر والبدانة على ضخامة البطين الأيسر.
أجريت الدراسة على /150/ مريض داء سكري من النمط II خلال عام 2003 في مشفى الأسد الجامعي، فأجري تصوير بالأمواج فوق الصوتية للقلب عند جميع المرضى الذين قُبلوا في الدراسة وتراوحت أعمارهم بين 35-73 سنة وتم تقسيم المرضى إلى فئات عمرية وكذلك حسب مشعر كتلة الجسم BMI وحُددت نماذج الضخامة ونسبتها في كل مجموعة.
النتائج: شملت الدراسة /150/ مريض (70 ذكر و80 أنثى) متوسط أعمارهم 52.2±9.26 سنة. ومتوسط BMI 24.9±8.1. وكان مشعر كتلة البطين الأيسر لكامل العينة LVMI (110.5±24.5 غ/م2).
حيث وجد أن 30.6% من المرضى لديهم ضخامة بطين أيسر وذلك عند المرضى السكريين بدون بدانة وارتفعت النسبة إلى 49.3% عند المرضى السكريين مع بدانة.
كذلك لوحظ ازدياد حدوث ضخامة البطين الأيسر مع تقدم العمر.
وكذلك لوحظ غلبة ضخامة البطين الأيسر من النموذج (Eccentric Hypatrophy) EH (الضخامة غير المتراكزة) (58.3%) تليها (Concentric Remodelling) CR (26.6%) ثم CH (Concentric Hypertrophy) (15%).
Purpose: To evaluate left ventricular hypertrophy and ventricular geometric pattern, and the impact of age and obesity on LVH in patients with II diabetes mellitus.
Left ventricular hypertrophy was studied at 150 patients with type II diabetes mellitus in AL-ASSAD University Hospital in Lattakia during 2003.
Echocardiography was performed to all the patients whose age was between 35-73 year and we had divided them into age-groups and according to BMI and the ventricular geometric patterns and ratio were defined in each group.
Results: The study was carried out on 150 patients (70 males, 80 famels) mean age was 52±9.26 years. And the mean BMI was 24.9±9.26, and LVMI was 110.5±24.5 g/m2.
30.6% of non obese diatetics have left ventricular hypertrophy, and this ratio increases to 49.3% of obese diatetics. LVH also increased with aging.
It was noticed that EH pattern (eccentric hypertrophy) is the most common pattern (58.3%), then the CR (Concentric Remodeling) pattern (26.6%), and then CH (Concentric hypertrophy) (15%).
