إصابات الأوتار المثنية في اليد
Abstract
لقد هدفت هذه الدراسة إلى تقويم نتائج علاج المرضى المصابين بأذيات رضية في الأوتار المثنية على مستوى الأصابع وراحة اليد و الساعد وذلك من خلال دراسة النتائج الوظيفية لعلاج واحد وثمانين مريضا من الذكور و الإناث قبلوا للعلاج في مشفى الأسد الجامعي خلال عامي 1999-2000تراوحت أعمارهم بين 10-73 سنة.
اعتمدت الدراسة على طريقة Boychev لتقويم النتائج وذلك بعد مضي فترة لا تقل عن ستة أشهر من تاريخ الجراحة وذلك بعد تقدير حجم حركة الثني الفاعل في مفاصل الإصبع المصابة ومقدار تحدد حركة البسط في نفس المفاصل.
لقد أكدت الدراسة أن جودة النتائج الوظيفية بعد جراحة الأوتار المثنية في اليد تتعلق بمجموعة عوامل أهمها مستوى إصابة الوتر وعدد الأوتار المصابة وتطبيق الجبيرة الديناميكية واللجوء إلى العلاج الفيزيائي في الوقت المناسب في حين لم تلعب عوامل أخرى كالعمر والجنس دوراً يذكر.
إن من أهم عناصر الحصول على نتائج وظيفية جيدة بعد خياطة الأوتار المثنية في اليد هو تحقيق خياطة متينة بين طرفي الوتر المقطوع بأقل كمية ممكنة من الخيوط غير القابلة للإمتصاص والبدء بالتحريك الباكر قدر الإمكان والوقاية من حدوث الإنتان الذي يسيء للإنذار في حال حدوثه.
This study estimates the results of treating patients with flexor tendons injuries in fingers, palm and foream. We have studied functional resultes of 81 patients admitted to Al-Assad Hospital during 1999-2000; Their ages were between 10-73 years.
We depend on Boychev method which offers resultes after 6 months of surgery, by estimating the range of active flection in jointes of the affected fingers and the range of extention restriction at the same jointes.
The research reassures that the good functional results after surgery is related to many important factors such as type of tendon injury, number of affected tendons, using the dynamic plaster and the physiotherapy at suitable time; While factores like age and sex didn't play important role.
One of the most important factors to have good functional results in suturing tendons is to make firm suture between tendon ends using the least amount of unabsorpable threads, to begin movment as fast as possible and to prevent infection.
