الأهمية التشخيصية والعلاجية لتنظير الركبة في الأذيات الرضية للغضاريف الهلالية
Abstract
لقد شملت الدراسة على 134 مريض في مشفى الأسد الجامعي باللا ذ قية ممن أجري لهم تنظير ركبة بين عامي 1999-2003 وذلك بتشخيص بدئي : شك تمزق غضروف هلالي بالركبة أو تبين لدى اجراء تنظير ركبة لديهم وجود أذية رضية للغضاريف الهلالية.
تكمن أهمية الدراسة في ان الاصابات القابلة للعلاج تعطي أعراضا قد لا تكون في البداية معيقة للحركة ثم تسير الاصابة بالتدريج لتنتهي بالتبدلات اللا عكوسة في المفصل وحينها يكون العلاج صعبا جدا ومتأخرا والنتائج الوظيفية سيئة ومن هنا تأتي أهمية تطوير طرق تشخيص وعلاج هذا النموذج من الاصابة مع دراسة واقع الحالات التي تراجع المشفى بتأخر التشخيص والعلاج.
لقد قسمت عينة الدراسة الى مجموعتين:
1- مجموعة التدبير بالمرحلة قبل بدء التبدلات التنكسية وهي المرحلة التي يشاهد بها غالبية المرضى في المشفى مع النتائج التي تتلو هذه المرحلة أيضا
2- مجموعة التدبير بعد التبدلات التنكسية بالمفصل مع مقارنة المعطيات التنظيرية والسريرية بشكل عام ضمن كل مجموعة
لقد كان عدد المرضى الذكور 93 مريضا بنسبة 69.4% والاناث 41 مريضة بنسبة 30.6%
و يلاحظ شيوع الاصابات في سن النشاط (20-50 سنة)
The study includes 134 patients who underwent a Knee-arthroscopy in Al-Assad
University Hospital in Lattakia between 1999-2003 primary diagnosis includes that they have an anticipated tear of meniscu and arthroscopy points to a traumatic meniscual injury.
The importance of the study is in the fact that curable injuries include symptoms which may not limit the range of motion at the begining, as a result injury develops gradually giving unreversable changes of the joint, to make therapy difficult and the functional results very late .
For this reason it is necessary to developed advance of diagnosis Without neglecting late diagnostic and therapic cases.
The specimen of the patients is divided into two main groups :
1- Patients who had been cured before degenerated changes took place :the period in which most patients were available in the hospital
2- Patients who had were cured after degenerated changes took place
Moreover a comparison is made between the clinical signs and the diagnostic signs in each group.
According to gender, the number of males is 93 and the percentage is 69.4% ,whereas females is 41 and the percentage is 30.6% with a high incidence of the injury in the age of activity (20-50 year)
