العلاقة بين الانتان بالملتوية البوابية (Helicobacter pylori ) والقلس المعدي المريئي GERD))
Abstract
Helicobacter pylori infection has a worldwide prevalence dominant in developing countries. The role of H. pylori infection in gastroesophageal reflux disease (GERD) remains controversial. . This study aimed at finding the prevalence of H. pylori in patients referred to the GI departments in Syria and whether it has any protective role against the GERD.
We performed a prospective case-control study, in which the prevalence of H. Pylori infection was compared in subjects with and without GERD complain. The diagnosis was based on CLO-test and histopathological data.
A total of 93 subjects were enrolled with (n=49) and without GERD (n=44) mean age years.The subjects were highly infected with H. pylori and the colonization pattern was antrum-dominant (exceeds 60.2% in the antrum versus 41.9% in the body)
According to histological data and CLO-test, the prevalence of H. pylori infection was same in both the groups (60% vs. 59.1%, P=0.834 in the antrum; and 42.8%. 40.9%, P=0.849 in the body). The results indicate the absence of protective role of H. pylori against GERD.
ينتشر الانتان بالملتوية البوابية بشكل واسع عبر العالم وخاصىة ببلدان العالم الثالت . ولا تزال العلاقة ما بين الانتان بالملتوية والقلس المعدي المريئي موضع جدل . تهدف هذه الدراسة لمعرفة انتشار الانتان بالملتوية البوابية عند المرضى المراجعين للشعبة الهضمية في مشفى تشرين الجامعي في اللاذقية والبحث عن العلاقة ما بين هذا الانتان والقلس المعدي المريئي . شملت هذه الدراسة المستقبلية Case – control study) ) 93 مريض يشتكي 49 منهم من اعراض القلس المعدي المريئي و44 مريض بدون أعراض القلس . وسطي العمر 41,4 سنة . أغلب هؤلاء المرضى كان مصابا بالملتوية البوابية مع نسبة انتشار وصلت 60,2% بالغار مقابل 41,9بالجسم . وبحسب نتائج الدراسة النسيجية واختبار ال clo-test فإن معدل انتشار انتان الملتوية البوابية كان متساويا في كلا المجموعتين (60% مقابل 59,1% مع p=0,834 بالغار , و 42,8مقابل 40,9% مع p=0,849 بالجسم ). تشير هذه النتائج لعدم وجود دور واقي للملتوية البوابية من القلس المعدي المرئيي .
