التقييم السريري لمرضى انقطاع التنفس أثناء النوم
Abstract
تم تقييم 100 مريض يعانون من متلازمة انقطاع التنفس أثناء النوم، و ذلك من خلال أخذ القصة المرضية والأعراض السريرية و الفحص السريري, و خاصة بما يهم اختصاصنا, و هو فحص الأنف و البلعوم و البلعوم الأنفي و الحنجرة و اللسان،و أجري لقسم من المرضى تخطيط أثناء النوم و ذلك لتأكيد التشخيص، بينت الدراسة أن الشخير و النعاس النهاري و الاستيقاظ من النوم هي أكثر الأعراض شيوعاً لدى مرضى متلازمة انقطاع التنفس أثناء النوم، وأن ضخامة اللهاة مع ترهل شراع الحنك و ضخامة كتلة اللسان FTP3 حسب تصنيف Friedman وضخامة اللوزات الدرجة TS3 هي أكثر الموجودات السريرية شيوعاً في هذه المتلازمة، كما أن السمنة و حجم العنق تعطي فكرة هامة عن شدة المرض، و يأتي تخطيط النوم وسيلة ذهبية لتأكيد التشخيص, و ذلك بإعطاء معلومات عن وجود انقطاع النفس أو ضعف النفس و الشخير كما يقيس معدل جريان الهواء و دراسة حركات الصدر و البطن.
One hundred patients with obstructive sleep apnea syndrome (OSA) were evaluated in this study, after taking clinical history and symptomsespecially which are related to Otolaryngology, by nose, nasopharyngeal, throat and tongue mass exam.
The study shows that the snoring, daytime sleepiness and arousal from sleep are the common symptoms,italso shows that the large uvula, low palatal arch, tongue mass FTP3 and TS3 are the most common clinical features of obstructive sleep apnea. Polysomnogrphy is important in the evaluation of OSA and to get a perfect diagnosis.
